Release and Transfer Nurse

As a Release and Transfer Nurse, your responsibilities will vary. You will

• As Release and Transfer nurse you will use your expert clinical skills and demonstrate judgement in assessing and managing wide ranging and complex patient problems. You will provide care and planning for adult male prisoners from the point of reception until the day of release or transfer to another establishment. You will be expected to work collaboratively with the patient and any other agencies to support a safe transition back into the community/ other establishment. 

• Your role will consist of managing face to face clinics with patients and being the experienced clinician for patients who may need a little more support for their release/ transfer. You will work closely with GP colleagues for those cases that need further nursing input. You will request TTO’s for those patients leaving the establishment and ensure they have a discharge letter and/or letters for any future hospital appointments they may have

• Registered Nurse: with current NMC registration 
• Experience demonstrating high levels of clinical expertise in decision making and judgement, enabling the development, monitoring and improvement of standard of care through supervision, clinical practice, audit, research, education and professional leadership 

Although not essential ideal applicants would have experience in a hospital, community or prison environment.
