Practice Plus Group are looking for clinicians for the new Unscheduled Care Coordination hub (UCCH) in Suffolk and North East Essex.
To be successful in this role, you will need to hold good knowledge of local urgent care, primary care and secondary care pathways and infrastructure. You will need to collaboratively work with the SNEE system partners to help deliver better quality of care for patients.
This is the perfect chance to use and develop your skills and expertise to provide quality, evidence-based health care to meet the immediate needs of our patient population.
The service is currently operational 7 days a week, 0800hrs-2000hrs with opportunities for full or part-time work either salaried or bank. This is initially a fixed term contract for a year with the possibility of permanent employment at the end of this period.
This role is not suitable for home-based working unless as part of a business contingency measure.
