Practice Plus Group are looking for Palliative Care Clinicians (Specialist Doctors, GPs, ANPs and ACPs) to join a team of well established, experienced senior clinicians working across the Devon Integrated Care System (ICS) to a assist in the development of new models for service delivery that have a positive impact on the health economy.
You will consult with patients and representatives with palliative care needs over the phone, who present with urgent and unscheduled care needs. You will work within your sphere of competency to provide assessment of patients, plan their care, arrange treatment and refer as appropriate to other healthcare professionals / external agencies. This will include prescribing for patients as required.
