Social Prescribing Co-Ordinator

As a Social Prescribing Co-Ordinator your responsibilities will vary. You will

• Provide Leadership and management responsibilities for the social prescribing co-ordinators on site.
• Complete initial assessments with patients referred to and eligible for the social prescribing service.
• Assessing their current wellbeing and identification of what matters to them.
• Work with patients to develop personalised wellbeing plans based on services and activities that offer practical, social and emotional support that will improve their health and wellbeing.

Qualifications required

• A positive, inclusive and compassionate practitioner.
• Experience of using SystmOne
• Experience offering an information, advice and guidance service in a health, care or similar setting
• Experience of working within a prison environment or working closely with patient’s with challenging and complex needs.
• Experience of managing own caseload and understanding how to prioritise patient need and manage a complex caseload.
