Liaison and Diversion Practitioner

• Qualified Registered Mental Nurse, Mental Health Social Worker or Occupational Therapist with current registration with the relevant professional body 
• Substantial evidence of continuing professional development and post registration training. 
• Experience of working with patients, face to face, who have complex mental health conditions 
• Experience of psychosocial interventions and awareness of substance misuse    
• Excellent communication skills and ability to gain appropriate level of consent  
• Understanding of clinical governance, health and safety, QOF, EBP and national standards 
• Knowledge of evidence based practice, health promotion, NICE guidelines and pharmacology     
• Knowledge of current legislation relating to community mental health provision and forensic psychiatry

We will position the right people in the right place at the right time, so that those whom are more vulnerable don’t risk being faced with a pathway that would be detrimental to their rehabilitation into society.

Our model is there to support individuals throughout their journey, and we will have a system in place to assist those who have completed their prison sentence and are re-joining the community.
