As a Mental Health Transfer Senior Practitioner, your responsibilities will vary.

• You will provide safe and effective care pathways for all offenders identified for transfer, under Section 47, or Section 48, and Hospital Order, to external NHS or independent in-patient services, for those being remitted back to prison and to support the London-wide Service Lead
• Achieve all key related strategic objectives set out in the National Guidance and equivalent
• You will work alongside the responsible prison clinical teams, both the Integrated Mental Health Team and In Patient Unit
• Maintain accurate and timely transfer activity, information, and data on the London Regional Mental Health Transfer data tracker system
• Support all future developments through SystmOne
• Supporting Local, Regional and National Reporting
• Support effective liaison with providers and commissioners to ensure funding decisions are expedited to ensure transfers take place as required to meet all key targets, in relation to Compact and Who Pays Guidance.

Qualifications required: 

• Registered Nurse (RMN): with current NMC registration
• Evidence of implementation of local and national policies
• Working within MDT 
• Experience of MHA Transfer, Criminal Justice System interface and knowledge of Prison Health Care
• In-depth knowledge of the Care Programme Approach (CPA) and Mental Health Acts
  

Although not essential ideal applicants would have experience in a hospital, community or prison environment.
