As a Learning Disability Nurse with both mental health and learning disability experience you  will be joining a team that provides a stepped care approach offering In-reach and Forensic models of engagement similar to those experienced and replicated within the wider community. As the Complex Care Nurse within our team you will be involved in co-ordination, screening, assessment, planning, implementation and evaluation of individual Patient programs of care. Managing a caseload of patients deemed to have mild cognitive impairments, a diagnosis of ASD, ADHD with a focus on risk assessment and risk management strategies, ensuring that the requirements of the Care Program Approach are met in a timely manner. You will working with multi-disciplinary care planning, both within the clinical setting and also in partnership with prison. You will  also be involved in supporting group interventions and raising awareness about the mental and physical health needs of adults who have a learning disability. 

About You

You will be a qualified Registered Nurse (Mental Health) with a real passion for developing your clinical skills in an environment that is ever changing. You’ll have a flexible approach working within a team that encourage new ideas and opinions on how to improve the service.

You’ll receive an annual salary up to £39,300 per annum - depending on experience.
