About The Role

The Admissions and Discharge Nurse will work as part of a nurse led team, within a wider multidisciplinary healthcare team. The principle aim of this post is to ensure patients are safe on their first night in prison; by undertaking a comprehensive assessment of their physical, mental, substance misuse and social care needs. Risk assessment skills are essential, as you will be the first point of contact within the prison and will assess the patients risk to themselves and others.

The discharge process is fundamental to the through care of our patients back into their communities. Your key function as part of this process will be to ensure the patient has appropriate take home medication, informed of any appointments with external agencies and play a key role in sharing communication with external agencies regarding the patients ongoing care needs.




About You

You will have a real passion for developing your clinical skills in an environment that is ever-changing. If you are self-motivated and able to work autonomously as well as showing plenty of initiative when it comes to handling a wide range of situations, then this could be the right role for you!

You should possess a good working knowledge of conducting risk assessments and care planning as well as someone who is creative and forward-thinking to work through the issues that may arise in this challenging but exciting environment. Knowledge in therapeutic interventions such as CBT or Steps to Wellbeing is desirable but transferable clinical skills will be considered with appropriate training provided.
 
This is a full-time role working 37.5 hours per week. The salary for this role is up to £33,600 per annum - dependent on experience (pro-rata for part-time hours).
