About The Role

In order to meet the needs of patients in our prisons, Practice Plus Group have developed a new integrated approach to the provision of care. Teams will work together at each stage of the patient journey, supporting them as they arrive in the prison (Early Days in Custody Team), delivering the care they need, promoting good health and supporting resettlement (Planned Care Team) and responding to urgent and emergency needs (Unscheduled Care Team).

As a Clinical Co-ordinator you will be working within the Early Days In Custody team. You will be responsible for ensuring that all patients who enter the prison receive all the required assessments and these feed into the patients overall care plans. You will be responsible for ensuring that reception screening is completed thoroughly and all second day screenings are done within 7 days for all new receptions. 

About You

You will work closely with the EDiC Clinical Lead and be responsible for those working in reception, First Night centre and the Conibere Unit. Your team will be made up of a variety of registered and non registered professionals including; mental health, and substance misuse practitioners. You will act as a role model ensuring professional appearance and maintaining professional standards at all times.
 
You will lead the daily hub meeting where all new receptions will be discussed and referred onto clinical pathways. You will participate in reviews of pathways and ensure that all patients discharged into planned care have an appropriate care plan in place.
