About The Role

We are looking for Home Visit Clinicians committed to working as part of our OOH Team to deliver an excellent and caring service to our patients.

This text is address to GPs/ANPs/ACPs with recent experience in Primary Care or Urgent Care.

Integrated Urgent Care at Practice Plus Group provides urgent care via 111, Out-Of-Hours, GP and Clinical Assessment services on behalf for the NHS. Join us, and you'll be helping relieve some of the pressure on NHS Emergency services at a vital time.

Essential Responsibilities

Home visiting in a chauffeured car
To visit and consult with patients referred on the day by in-hours GP surgeries in North East Essex
Expectation to work flexibly in line with service pressure and demand
To represent Practice Plus Group in a professional manner when in contact with the general public and other professionals.

PERSON SPECIFICATION

Practice Plus Group Clinician

	Essential	Desirable
EDUCATION/ QUALIFICATIONS	GP Current GMC registration Full membership of a medical indemnity organisation Inclusion on a PCT Medical Performers list ANP/ACP Current NMC registration Completion of (or currently working towards) a Masters degree (or equivalent) Independent prescribing qualification Current HEP B immunity certificate BLS/ILS Level 3 Safeguarding Adults and Children	Experience of working within Primary Care or Urgent Care within the past 6 months Experience of Medical, Surgical, A&E, Palliative Care, Mental Health and Paediatrics ATLS/ACLS/PILS
SPECIALIST SKILLS/ ABILITIES	Basic computer literacy and keyboard skills Good telephone manner & clear diction. Timely record keeping. Excellent communication and listening skills. Team worker.
RELEVANT EXPERIENCE	Primary care home visiting, triage and face to face consultation Ability to demonstrate holistic patient care
KNOWLEDGE	An up to date understanding of modern primary care
COMPETENCIES	Self motivated and flexible Empathetic and caring Socially & ethically aware Professional approach and a presentable appearance Ability to work under pressure and manage a demanding workload
