About The Role

As a Social Care Support Worker, your responsibilities will vary. You will be involved in the implementation of patient care packages. You will support and assist patients with the activities of daily living, work to help them fulfil their daily tasks and you will be encouraged to take the lead on well-being promotion clinics and therapy sessions.

 You will support the clinical team and occupational therapist/physiotherapist in implementing agreed care plans, in association with the patient and in accordance with instructions and training received, whilst promoting safe patient well-being .You need to be willing to develop and apply new skills to maintain and enhance clinical service delivery.

HMP YOI Moorland is a category C men's prison and Young Offenders Institution based in Hatfield, South Yorkshire. The site offers a range of healthcare services including Physical Health, Mental Health, Pharmacy and Substance Misuse. The 9-bed specialist Intermediate Care & Reablement Service which opened in early 2022 based at HMP Moorland.

About You

We are ideally looking for someone who has experience of working in a social care support role or similar. You must hold a minimum of NVQ 3 in Health and Social Care or equivalent qualification and must have a willingness to undertake training in Mental Health Awareness, suicide prevention, self-harm and ACCT procedures, Health Promotion and screening, as well as, if not already achieved.
