About The Role

We would love to hear from clinicians that want to make a real difference to the lives of patients and support them during their early days in HMP Portland.
 
We're looking for an Early Days Mental Health Practitioner to work as part of a dynamic team. The post holder will help support a wide range of benefits to improve the welfare and service provided to prisoners who are moving through the custodial system. You will ensure that when a patient is transferred from a reception prison or another prison, the mental health team receives a comprehensive handover before the transfer takes place. You will identify immediate mental health needs including, any external services or prescribing requirements that prisoners may have received prior to arrival in the criminal justice system and, may need assistance with within the first few days of admission. You will enable timely access to mental health interventions and treatment and work to Multi-Disciplinary Team (MDT) working across HMP Portland and healthcare teams by enabling a fast-track approach to assessments and interventions, based on clinical need.

Where individuals are at risk of self-harm, you will ensure an appropriate recovery orientated Early Days care plan is put in place - One that focuses on delivering integrated seamless person-centred care aligned to the clinical and psychosocial service models already in place within the prison. You will help to ensure that prison and healthcare staff are appropriately informed, feel engaged and supported to enable them to reduce and prevent self-harm and suicide. There are many aspects to this exciting and varied role, working within a supportive, multi-disciplinary team within HMP Portland.

About You

You will be an approved Mental Health Professional with a degree in Healthcare or Social Work, CQSW/DipSW or equivalent. This could be a dual-qualified (MH and LD) Nurse as identified in the national specification. You will have significant diverse experience of working in integrated adult mental health services, and experience of complex mental health issues as well as experience of providing a consultative service to members of a multi-disciplinary on-site HMP, health or social care team. A good working knowledge of all relevant Mental Health and Social Care legislation and Codes of Practice.

HMP YOI Portland is based on the Isle of Portland off the coast of Weymouth. It is approximately 7 miles from the centre of Weymouth. The prison is a category C adult /young offender’s prison and houses up to 530 prisoners, all with varying sentence lengths.
