Complex Care Nurse

As a Complex Care Nurse your responsibilities will vary. You will

• Liaise with internal and external stakeholders to ensure a safe and effective transfer into the prison setting with the appropriate care package in place
• Work with the Clinical Hub specialist in the proactive management of LTC identifying patients
• Develop and formulate a “Complex Care Register” for the region
• Lead on the development of the RMPCC process

Qualifications & Experience required

• Registered Adult Nurse
• Current professional qualification (NMC)
• A minimum of 2 years’ experience at a Senior Staff Nurse level (Band 6 NHS equivalent)
• Specialist interest in care of the older person/Complex patients
• Experience of managing your own caseload
