About The Role

As a healthcare provider, we recognise the fact that prisoners are a socially-excluded group with complex healthcare needs that can often be associated with poor outcomes. We believe in putting the patient first, regardless of the environment or their history.

You'll enhance the provision for the process of the patient discharge from health care in a custodial setting to healthcare in a community setting by accessing and navigating the appropriate primary care and community services and that this is as seamless as possible for the individual.

You will also aim to educate and promote patient confidence about the management of their condition, and their own responsibilities; fostering patient empowerment to make appropriate health choices and access the relevant services.

Liaising with the prison and health partners to identify at the earliest opportunity those prisoners with health and/or social care who are due for release and require health plans in place (physical and mental). You'll develop, maintain and promote effective communication (information flow/sharing, processes, governance) between the internal prison services and the external services. e.g. GPs, Pharmacy, Mental Health services, social services, probation service, any Intensive Integrated Risk Management Services (IIRS) etc. in each of the areas the role covers, developing and maintaining a ‘directory of services’.

You will receive a salary of up to £38,733 depending on experience. We recognise training and development is key, therefore we training centre to facilitate your needs. You will gain insight and training in Mental Health and Substance Misuse.
In this environment, we appreciate flexibility around working hours hence we are willing to accommodate part-time requests. We offer a competitive annual salary plus payment of MDU/MPS subscription and CPD days.

About You

As a registered healthcare professional (Registered Nurse, Occupational Therapist for instance), you’ll be an integral part of the senior team at HMP Dartmoor, bringing your well-developed skills, passion for care, and leadership skills to the role.
With plenty of recent experience in community/acute healthcare, you'll have proven skills in assessing the needs of and working with individuals with complex health and social care needs.
You'll be able to demonstrate your experience of providing a consultative service to members of a multidisciplinary custodial, health, or social care team.
To this role, you'll bring your first-rate coordination and communication skills, as well as proven knowledge of clinical governance and evidence-based practice.
