You will be required to lead innovative nursing practice in the management of long term conditions for a specific caseload of patients. This includes the assessment of need, delivery, and evaluation of care interventions and management plans in line with best-practice standards.

You will also be required to proactively manage frequent and high demand users of health services using long term condition strategies in line with the Department of Health standards for the management of long term conditions as well as providing professional support and guidance for the nursing team and ensure resources available are utilised effectively for the day to day management of patients with long term conditions. The post holder is an integral part of the Service and contributes to the assessment of the needs of the local population.

You’ll receive an annual salary of up to £39,500 FTE - depending on experience; free DBS check, free uniform, free mandatory training, indemnity cover, CPD, and re-validation support.

Examples include ANP, BBV, CBT, sexual health, plastering, ILS (intermediate life support), health promotion, phlebotomy, vaccinations, suturing, wound care, RCGP, and long-term condition courses. As well as team and manager essentials training.

About You

As a Long Term Conditions Nurse, you’ll be an integral part of the healthcare team by bringing your top-notch clinical skills and passion for care. You will have highly developed specialist knowledge in long-term conditions and be able to demonstrate a high level of clinical assessment skills. You will also need to undertake the role of Supervisor/Mentor, participating in and supporting others to deliver clinical practice.

You will also require a non-medical prescribing qualification and have experience of working in integrated adult or older person services.

HMP Isle of Wight is a category B prison that is based over 2 sites Albany and Parkhurst and has a population of 1100 prisoners.

Our two sites-based Primary Healthcare Centres provide a comprehensive primary care service, GP appointments, chronic disease nurse-led clinics, screening programmes, and health promotion work. An emergency response service to prison wings is also provided.

There is also an Inpatient Healthcare unit (IHU) on the Albany site which provides 24-hour care to those with mental health, physical health, substance misuse, and palliative care needs. The post-holder may be expected on occasions to rotate to the inpatient unit.

Additional Information
This is a full-time role, working 37.5 hours per week. The salary for this role is up to £40,500 per annum - dependent on experience.

This is a fixed-term contract for 12 months.
